1. Are you an Australian citizen or permanent resident of Australia aged 18 years or over?
Yes No

Note: You must be an Australian citizen or permanent resident aged 18 years or over to qualify for the program.

2. Have you claimed the Australian Government's Solar Hot Water Rebate since 3rd Feb 2009 or already claimed a rebate under the insulation program?
Yes No

Note: If you have already claimed either of these rebates you are no longer eligible for the program.

3. Are you the owner of the dwelling where the insulation is to be installed?
Yes No

Note: If you are not the owner of the dwelling then you are not eligible to apply for the program. However you may be eligible for the Solar Hot Water Rebate which is available to owner occupiers, landlords and tenants. 4. Is the residence where the insulation is to be installed your principle place of residence?
Yes No

Note: If you don't live at the address you are not eligible for the program. However you may be eligible for the Low Emission Assistance Plan for Renters. For more information and full terms and conditions please visit http://www.environment.gov.au/energyefficiency/insulation/

5. Is the dwelling a new home or a major extension of an existing home completed in 2003 or later?
Yes No

Note: Homes built from 2003 onward are not eligible for the program because they are covered by mandatory thermal performance requirements for new housing under the Building Code of Australia. 6. Does the dwelling already have insulation less than 25mm thick (2.5cm or 1 inch)?
Yes No
Please ensure that this information is correct as the installer will not be able to proceed with installation if your existing insulation exceeds this measurement.

Note: If your existing ceiling insulation is more than 25mm thick (2.5cm or 1 inch) thick then it will have a R-value greater than 0.5 and does not qualify for replacement under the program guidelines.
